Amanda Hall-Warburton, LMHP, CPC, CSW, (Licensed Mental Health Practitioner, Certified Professional Counselor, Certified Social Worker) provides a variety of services for our residents and patients including:

  • Case Management
    (linking clients with agencies and programs that will meet their psychosocial needs)
  • Community Organizing
  • Patient & Resident Advocacy

Advanced Directive

The links below direct you to some forms that allows you to make decisions about your personal, emotional, and spiritual needs as well as your medical wishes. For anyone 18 and older, you can choose the person you would like to make your health care decisions if you are not able to make them yourself.  By completing and signing the document, you can have the peace of mind knowing it substantially meets the state’s requirement under the law.  Below are a couple of documents to help you make important health care decisions:

Counseling Services

Amanda also provides Individual, couples, and family counseling on an outpatient basis. If you need help coping with any of life’s problems, therapy can help. Kearney County Health Services can provide confidential and professional counseling to individuals, families, and couples whose personal problems have become difficult to manage alone.
