For anyone 18 and older, you can choose the person you would like to make your health care decisions if you are not able to make them yourself.  By completing and signing the document, you can have the peace of mind knowing it substantially meets the state’s requirement under the law.  Below are a couple of documents to help you make important health care decisions:
